  • Pre-activity form: Info and download

Before you attend any sessions you need to complete a pre-activity form with a few questions about the state of your health and fitness and who to contact in the case of an emergency.

If you have any conditions that may be affected by exercise, you may be asked to consult your doctor and obtain a written medical clearance before you attend. This would be for things like heart problems, stroke, asthma, diabetes and arthritis.

Download the form, or pick one up at a council office or you can complete one when you turn up for a session–the instructors will have them on hand.

Council will collect the forms and they will be treated as confidential and only revealed if there is a safety issue.
